To save time during your first appointment, we have provided convenient forms that you can fill out in advance. Simply click here to access the necessary forms. You can then either email them back to our office or print them out and bring them with you.
What to Bring to Your Appointment
For a smooth and efficient visit, please bring the following items to your scheduled appointment:

MRI Films, CAT Scan reports, and X-Rays

Medical reports related to your condition

A list of all medications you are currently taking

Your insurance cards

Names and addresses of referring and primary doctors

Completed patient information forms (if possible)
Important Note about Blood Thinners
If you are currently taking any blood thinner medication such as Coumadin or Plavix, please inform us prior to any procedure. Your safety and well-being are our top priorities.
Conscious Sedation Information
For procedures requiring conscious sedation, please keep the following in mind:

Arrange for an escort to drive you home after the procedure.

Refrain from eating or drinking anything after midnight on the day before the procedure.
